Insurance Information

Dr. Stacey Munro is an in-network provider for Aetna, Anthem Blue Cross Blue Shield, Cigna, and Connecticare insurance. Co-payment or coinsurance is required at each visit. If you have an unmet deductible, full payment for services must be paid at the time of the visit. Uncovered services, such as lifestyle education and homeopathy, are not billable to insurance and may be charged separately.

For patients with Medicare, Medicaid, United Health Care, Health Net, Oxford, and Heath New England, or another unnamed insurance, full payment is expected at the time of the visit. You may be able to receive partial reimbursement for services. Check directly with your insurance provider about an out-of-network benefit.

Some insurance companies require prior approval for laboratory testing. It is your responsibility to determine your coverage for lab testing. The cost of an office visit does not include pharmacy items and laboratory testing.
